What forms will you need?

All clients need the New Client intake form.

Pregnant Women also need to fill out the Prenatal Form, and the Prenatal Physician's Permission to make sure your attending physician is alright with further treatment.

Cancer patients can only be seen if they are in Remission  and will need the New Client Intake form, Oncology Form, and Physician's Permission form
